Patient Participation Group

The Sandringham Practice is looking to set up a patient group, in order to seek the views of our registered patients.

We want to ensure that views of patients are fed in to the Practice regarding the services we deliver and any new changes or services that may be considered.

To do this we are compiling a contact list of email addresses so that we can contact you by email periodically to seek your views with a few questions.

If you are please provide your contact details via the form linked at the bottom of this page by 31 December 2011; we will only use the information to contact you concerning Patient Group matters and keep your details secure.

Survey information will be shared with participants and also displayed on our website.

Whilst the format will be discussed in greater detail in the near future, it is anticipated that most feedback will be provided via email.
